Adding Checklist Questions

The PSSR Checklist items are answer driven, meaning depending upon the answer the user may have to answer additional items.

To add a checklist item, simply find the checklist that was created and Click View Questions. If the user selects an existing PSSR checklist that is active a copy will be made and set in a draft status.

A new window will open and the User will see a grid of Checklist Questions.

  • Question: this is a question that may have a follow up question. The User would select the Question and the Response.
  • Response: the response may be a radio button, checkbox for multi select, drop down or text. The response will be where the user adds a follow up question (child question).
  • Category: this is a filtering mechanism in order to supply the Responsible Person of the MOC with Checklist Items that are appropriate for any particular MOC. This functionality is described below. Question is required, Attachment is required: checking this box as Required the user must address or attach documentation. These items will be a bold font if the question is required.
  • Use MOC Target Date: if this box is checked an Action item created from a PSSR Item will use the MOC Target Date. If this is not selected, the action items will be assigned the same Target Date as the PSSR Items.
  • Assignee: this field allows the Administrator to assign either a default User or a default Job Type to PSSR Items. Select the Assignee Type – either Job Type or Person. If Job Type is selected, a list of all Job Types in the database is displayed. If a Job Type is selected as the default Assignee, a list of all Users assigned to the selected Job Type AND assigned to the Location where the MOC resides is displayed when the PSSR Item is assigned to the MOC. If Person is selected, choose a name for the Default Assignee from the list, the User selected appears by default as the Assignee of the PSSR Item when the Item is assigned to an MOC. The Assignee can be changed as the list displayed by clicking on the drop-down arrow to display a list of all Users with permissions assigned to the Location.
  • Question Text
  • Response Type: this is the type of control radio button (Yes, No) can only select one, select drop down (list of items) can only select one, checkbox (list of items) can select multiple, Date/Time, Numeric (number only) and text response (type in comments). Response Type character limits are 250 characters.

If the user has selected a response type of radio button, select or check box and does not enter a response, the user will have to enter comments in the checklist item in tracker.

Checklist Question Example

Do the drawings need to be updated? (Parent Question) Yes, No If this is a Yes (Parent Response), then the follow question or item would be, list the drawings in the box that will need to be updated.

To add a checklist question, the first one will be added to the question text. This can then become a parent for the next question if desired.

  1. Click the Add Question button
  2. Enter the question into the question text field
  3. Is the question or attachment required. Click the box if either of those items are required
  4. Click the Use MOC Target Date if the user would like to use the MOC target date for the Action item created
  5. The question can be assigned by Job Title or to a Person
  6. Use the select for the category - multiples may be selected

If no category is selected the checklist will be available to use in the PSSR Checklist templates in Resolution Tracker for that location.

  1. Using the drop down, select the Response Type
  2. Click Save
  3. Click on the Question (blue circle)
  4. Click on Add Response button

For each Response the user will click the Add Response button.

  1. Enter the Response text
  2. Click Save

The question will be saved and be displayed in the Question grid. The grid consists of:

  • Question Text: the user's question text
  • Response: the user's answer text.

Adding Child Questions

Adding a Child Question, follows much of the above with two additional fields being used.

  1. Click on the response. The question and response will become bold and the Add Question button will be active.
  2. Click on the Add Question button. A question window will open.

Acceptable Response Type shall be Radio Button, Select Drop Down and Check box only to add a child question.

  1. Add the question text.
  2. Is the question or attachment required. Click the box if either of those items are required
  3. Click the Use MOC Target Date if the user would like to use the MOC target date for the Action item created
  4. Use the select for the category

If no category is selected the checklist will be available to use in the PSSR Checklist templates in Resolution Tracker for that location

  1. Select either the Job Type or a Person as an Assign To
  2. Select the Response type using the drop down
  3. Click the Save button to add question to the checklist.
  4. Click on the Question. The question and the parent question will be in bold.
  5. Click Add Response button. The Response window will open.

For each Response the user will click the Add New Response button.

  1. Enter the Response text
  2. Click the Save button. The response is saved under the question.

Editing a PSSR Checklist Question

To edit a question in the checklist.

  1. Navigate to the Evaluation Checklist
  2. Click View Questions.
  3. Select the question to edit by clicking on the question
  4. Make the desired changes.
  5. Click the Save button to Save the changes to the database.
  6. If the question was part of an Active PSSR Checklist the question will be updated and the PSSR Checklist will be set with a Status of Draft. If the question was part of a Draft PSSR Checklist the status will remain the same. If the question was part of an Obsolete PSSR Checklist the question will be updated and the PSSR Checklist will be set with a Status of Draft.

Deleting a PSSR Checklist Question

To Delete a question select the question and click on the question, then click the Delete button located to the right of the Save button in the question.

If the user deletes a Parent Question all associated child questions and responses will be deleted as well.
