OII Checklist Questions

Adding Checklist Questions

The OII Checklist items are answer driven, meaning depending upon the answer the user may have to answer additional items. To add a checklist list question click on View Questions.

A new window will open and the User will see Checklist Question.

  • Checklist Title: shows the user the checklist being worked
  • Parent Question: this is a question that the user has decided to have a follow up question. The User would select the Parent Question and the Parent Response.
  • Parent Response: the next question will be based on the response given in the parent question
  • Question Text
  • Response Type: this is the type of control radio button (Yes, No) can only select one, select drop down (list of items) can only select one, checkbox (list of items) can select multiple, Date/Time, Numeric (can only be a number) and text response (type in comments). Response Type character limit is 250 characters.

If the user has selected a response type of radio button, select or check box and does not enter a response, the user will have to enter comments in the checklist item in tracker.

Example

What PPE was worn? The items can be listed as a checkbox to allow more than one selection.

To add a checklist question, the first one will be added to the question text. This can then become a parent for the next question if desired.

  1. Click Add Question
  2. Enter the question into the question text field
  3. Check the box if the question is required or an attachment is required
  4. Select the Response type using the drop down
  5. Click Save
  6. Click the Question
  7. Click Add Response

For each Response the user will click the Add Response button. 9. Enter the Response text 10. Click the Save button

The question will be saved and be displayed in the grid below the Checklist Question.

The grid consists of:

  • Question Text: the user's question text
  • Response: the user's answers

Adding Child Checklist Questions

To add a child (follow up) checklist question, the user can follow these steps:

  1. Click on the Response (green square)
  2. Click the Add Question button
  3. Enter the question into the question text field
  4. Check the box if the question or attachment is required
  5. Using the drop down select the Response Type.
  6. Click Save
  7. Click on the question (blue circle)
  8. Click Add Response

For each Response the user will click the Add Response button. 10. Enter the response text 11. Click Clear to add additional question(s)

Editing Checklist Questions

To edit a checklist question, the user will need to go to the Administration>Checklists>OII.

  1. Select the checklist to edit the checklist items.
  2. Click the question to edit
  3. Make any changes to the questions
  4. The User may want to select a different Response Type
  5. If the user does want to change the Response Type, use the Delete button to delete current responses
  6. Click Save
  7. Click the question
  8. Click Add Response button (if the response type has changed)
  9. Enter the new/updated response
  10. Click Save

Deleting Checklist Questions

To Delete a question select the question and click the Delete button found to the right of the save button.

If the user deletes a Parent Question, all associated child questions will be deleted as well.
