📄️ Checklist
Investigator provides additional functionality that allows an administrator to set up Investigator Checklist items to determine whether or not any infractions to company principles or doctrine occurred that could have caused the Incident.
📄️ Chemicals
Investigator allows entering information relative to any chemical that was involved in the Incident in the Chemicals tab. These chemicals could have attributed to the cause of the Incident or could have been released due to the Incident.
📄️ Close Incident
During the course of approving an incident, there may be times that the incident does not justify starting an investigation or there could be duplicate incidents. The user can close an incident if the permissions are available (See Manage Role Based Security). The closing of an incident will be during the initial approval process and cannot be done after the investigation is put into an In Progress status.
📄️ Close Investigation
After all of the information has been entered into the Investigations, it can be closed. Under Record Options Click on the Complete Investigation. The Incident Status will go to Awaiting Final Approval. The User who has the "Finalized Report" token can Finalize the completion of the investigation. Once Finalized has been clicked under the Records Options, the Incident Status in Record Details will be Report Issued.
📄️ Costs
Investigator has added functionality that allows recording and tracking of specific cost information relating to an Incident and Investigation. The user can define categories or types of costs that are applicable to the location.
📄️ Create Incident
To create a new incident, click on the Add Incident button under the Record Options.
📄️ Detailed Report
To view a document of all the details surrounding an incident, click View Detail Report under the Record Options located on the left. This report will only give you the information that has been entered into the record. If no information was entered, that section will not be displayed in the report.
📄️ Equipment
The Equipment screen allows the user to attach specific pieces of equipment involved in the incident. Only equipment associated to the location to which the incident is assigned will be displayed. The equipment will be set up in the Administration Locations with a User that has the permissions to do so. The user is able to add equipment on an ad hoc basis with the token in investigator "Add-New-Equipment-Incident".
📄️ Impact
Other windows are available to the Initiator for data input when initiating an Investigation: Impact, Chemicals, Weather, Notifications, Attachments, Equipment and Investigator Checklist. The fields on these windows are not required for the Investigation to be initiated. Each tab can be related to an incident type. However, the User should enter as much data as possible when initiating an Investigation.
📄️ Injury and Illness
If the user has not implemented the OI&I Module, Injuries & Illness tab will be available based on the incident type selected and must be set up in the Administration.
📄️ Investigation Details
The details for the investigation can now be entered.
📄️ Lessons Learned
The Lessons Learned screen allows the user to record any lessons learned from either the incident or resulting from the investigation.
📄️ Notifications
The Notifications screen allows the user to capture people, agencies, and local or government officials who were contacted in connection with the incident. KMS does not notify the official agency; it is only used to record the notification.
📄️ Recommendations
The Recommendation screen allows the user to enter recommendations that result from the investigation.
📄️ Root Causes
Investigator allows the user to assign Caused to an Investigation. The only required fields are Causes and Description. The Causes and Methodology are set up in the Administration>Selections>Investigator Library.
📄️ Search and Report
This section will discuss the different ways to Search and Report for Incidents and Investigations.
📄️ Start Investigation
Once the incident has been submitted for approval, the supervisor may decide to begin an investigation. The supervisor or a user that has the permissions can Start the Investigation. To begin the investigation, click Start Investigation. The Status under the Record Details will change to "In Process" and the workflow is in the "Conduct" stage. The Details are now ready to enter.
📄️ Submit for Approval
After entering all incident-related information into the appropriate tabs, click Submit for Approval. This is located to the left of the Incident Record under Record Options.
📄️ Team Members
The Team Members screen allows the user to document individuals involved in the investigation. The Team Lead 1 and Team Lead 2 (if a two team investigation) will be listed under Selected Team Members.
📄️ Weather
The weather screen allows the user to capture any weather-related information that might have contributed to the incident.
📄️ Witnesses
Investigator allows the user to enter Witness information and their statements relating to an Investigation.
