Evaluation Checklists Questions
Adding Checklist Questions
The Evaluation Checklist items are answer driven, meaning depending upon the answer the user may have to answer additional items.
To add a checklist item, simply find the checklist that was created and Click View Questions.
A new window will open and the User will see Checklist Question.
Question
This is a question (blue circle) that the user has decided to have a follow up question. The User would select Parent Question and the Response.
Response
The next question will be based on the response (green square) given in the question
Category
This is a filtering mechanism in order to supply the Responsible Person of the MOC with Checklist Items that are appropriate for any particular MOC. This functionality is described below.
Question is required, Attachment is required
Checking this box as Required the user must address or attach documentation. These items will be in bold font if the question is required.
Use MOC Target Date
If this box is checked an Action item created from an Evaluation Item will use the MOC Target Date. If this is not selected, the action items will be assigned the same Target Date as the Evaluation Items.
Action Assign
If you wish, you can determine a default User who is assigned the Action Item, if generated from the Evaluation Item. The list of Users consists of the Responsible Person for the MOC, Unit Supervisor, and Question Assignee.
Assignee
This field allows the Administrator to assign either a default User or a default Job Type to Evaluation Items. Select the Assignee Type – either Job Type or Person. If Job Type is selected, a list of all Job Types in the database is displayed. If a Job Type is selected as the default Assignee, a list of all Users assigned to the selected Job Type AND assigned to the Location where the MOC resides is displayed when the Evaluation Item is assigned to the MOC. If Person is selected, choose a name for the Default Assignee from the list, the User selected appears by default as the Assignee of the Evaluation Item when the Item is assigned to an MOC. The Assignee can be changed as the list displayed by clicking on the drop-down arrow to display a list of all Users with permissions assigned to the Location.
Question Text
This field allows the user to enter the question text
Action Tab
Will display fields that allow the user to enter default text that is used to populate the Action text field from an evaluation item. This text becomes the Action Description. This action would need to be fairly generic since it would be applied to all MOCs assigned to the specific Evaluation checklist question.
Response Type
This is the type of control radio button (Yes, No) can only select one, select drop down (list of items) can only select one, checkbox (list of items) can select multiple, Date/Time, Number (number only) and text response (type in comments). The response character limit is 250 characters.
Example
Do the drawings need to be updated? (Parent Question) Yes, No If this is a Yes (Parent Response), then the follow question or item would be, list the drawings in the box that will need to be updated.
To add a checklist question (Parent), use the following steps:
- Click the Add Question button located in the upper right of the window
- Enter the question into the question text field
- Is the question or attachment required. Click the box if either of those items are required
- Under the user can select either a job title to use, or a person with permission that is in that location
- The user may decide to select a assign dynamic (which is a role of Responsible Person or Initiator)
If no category is selected the checklist will be available to use in the Evaluation Checklist templates in Resolution Tracker for that location.
- Select the Response type using the drop down
- Click Save
Adding Checklist Question Response
- Click the question to add a response and press the Add Response button located to the left of the Add Question button
When the user clicks on the question, the Question and any associated parent question will become bold and the blue circle and green square will be enlarged.
If the user has selected a response type of radio button, select or check box and does not enter a response, the user will have to enter comments in the checklist item in tracker.
- Type in the Response Display Value
- Click Save
For each Response the user will click the Add Response button and the Save button.
Add an Action to a Checklist Question
There can be multiple Actions assigned to a question. Based on a response the action can automatically be assigned. Actions are the Red triangles facing upward. If there is an action to the question, the user may default assign it to someone by following:
- Click on the Action tab
- Enter the Action Text
- Click the drop down to select the Action Assignee can be Responsible Person, Unit Supervisor, Question Assignee or Initiator
- Click the drop down to select the Action Target Date, the Action Target Date can be None, Target Closure Date, Target Startup Date or Actual Startup Date.
- Use the radio button to determine + days after the action target date or - days before the target date and enter a number
- Check the box if an Attachment is required.
- If wanting to add to a specific step, click Add to Step, use the drop down to select the workflow step. Based on the response the action will be assigned to the workflow step.
- Additional Actions can be added to the question.
- Click Save
The question will be saved and be displayed underthe question.
Trigger a Workflow from a Question Response
Based on a response to a question, a workflow can be added to the MOC. The workflow that is added is tailored and must not have a proposal step.
To trigger a workflow follow these steps:
- On the Response, click Trigger Workflow
- A drop down is presented. The new workflow can be identified by the yellow star. Select the workflow from the drop down.
- Click Save
Adding Child Questions
Adding a Child Question, follows much of the above with two additional fields being used.
- Click the Response (the green square will become larger)
- Click the Add Question button, the question (blue circle) is now attached to the response
- Enter the Question Text
- Is the question or attachment required. Click the box if either of those items are required
- The user can select either a job title to use, a person or assign dynamic (for the parent it will only be Responsible Person) with permission that is in that location
- Select the Response type using the drop down
- Click Save
On the newly added queston, to add the response, follow the steps:
- Click Add Response
- Enter the Response text
- Click Save
Repeat the above steps for each response entered.
If no category is selected the checklist will be available to use in the Evaluation Checklist templates in Resolution Tracker for that location.
For each Response the user will click the Add Response button and the user must save after each response.
Editing an Evaluation Checklist Question
To edit a question in the checklist:
- Navigate to the Evaluation Checklist
- Click Select - the checklist must be in draft to edit a checklist question
- Click on the question to edit
- Make the desired changes and click the Save button
- If the question was part of an Active Evaluation Checklist the question will be updated and the Evaluation Checklist will be set with a Status of Draft. If the question was part of a Draft Evaluation Checklist the status will remain the same. If the question was part of an Obsolete Evaluation Checklist the question will be updated and the Evaluation Checklist will be set with a Status of Draft.
Deleting an Evaluation Checklist Question
To delete a question, the user will open the question. At the bottom of the question will be a delete button. The user can click on delete to delete the question and any associated child questions and responses.
