Create Risk Assessment

After the set of the methodology categories and the Risk Assessor methodologies are populated in the Administration, the user is ready to create a risk assessment in VisiumKMS.

Use the following steps to create a risk assessment:

  1. Under Record Options, located to the left of the My Assignments, click Create New.
  2. The Project Definition will open and the following fields are available for data entry:
  • Title (required) - This is the title of the risk assessment.
  • Prefix (required) - Each risk assessment has a unique alpha numeric prefix.
  • Location (required) - The location is a tree view of hierarchy. The default will be the user Primary Location, otherwise the user can select a different location.
  • Facilitator - This is considered the responsible person for the purpose of the risk assessment. The user can select a person from the pick list associated to the location.
  • Scribe - This is the person who will be taking the notes during the risk assessment. This can be the same person as the Facilitator, but at times may be a different person.
  • Scope (required) - Enter the scope of the risk assessment. This field can hold 4000 characters and has a spell check button to the left of the field.
  • Objectives - Enter the objectives of the risk assessment. This field can hold 4000 characters and has a spell check button to the left of the field.
  • Executive Summary - Enter the executive summary of the risk assessment. This is usually done at the end of the risk assessment, however, this can be completed as the risk assessment proceeds. This field is also 4000 characters and has a spell check button to the left of the field.
  • Start Date - This is the date the risk assessment is to begin.
  • Target Date - Is the estimated date that the risk assessment will be completed.
  • Draft Report Date - Is the date that a draft report will submitted approval.
  • Final Report Date - Is the date that the report has been submitted for final approval.
  • Revalidation Required? - This checkbox designates that a revalidation risk assessment will be needed.
  • Based on Date - This is a drop down to select when the revalidation should occur based on the date. The following are the choices available:
    • None
    • Start Date
    • Target Date
    • Draft Report Date
    • Final Report Date
  • Save - Will save the risk assessment
  • Cancel - Will cancel the risk assessment.

Delete Attendance

To delete an attendance item do the following:

  1. Select the date to be deleted and click on the X to the right of the date.
  2. A confirmation message will be displayed. If the user wishes to click OK, otherwise click Cancel.
